The term “Our Office” refers to the major organisations of Natural Lore Wellness, Church of Ubuntu, Ubuntu Wellness Clinic Newcastle, theportal.life and all subsidiary Certified Practitioners and/or associated Clinics.

Our office is dedicated to providing services with respect to human dignity. Protecting your privacy and your health information is fundamental in the course of our relationship. This notice provides an explanation as to how we will collect information and what we will do with your protected health information (all personal, financial and health information). This protected information may be received from you, your practitioner, or any other source in the normal course of our operations.

The protected health information provided to us may be used for treatment, payment, and other operations. It may not be disclosed for any purpose without prior written consent from the patient unless otherwise required by law.

DATA COLLECTION

The mission of Our Office, to improve the health and quality of life for you, our client, requires the ongoing collection and analysis of evidence-based research to ensure we are continually advancing our services and provisions to offer the best possible interventions for all concerned.

We routinely collect and manage client data in the form of an electronic health record. Ongoing analysis of this data enables us to establish a foundational platform of evidence-based protocols to better serve the needs of both Client and Practitioner.

Your participation in this program may not only assist your own wellness journey but may be of great benefit to others who are experiencing similar health issues. Collection of data enables us to:

-        Gather direct knowledge of the efficacy of cannabis formulas for a variety of conditions

-        Confidently provide protocol recommendations for a variety of conditions and situations

-        Collect in-depth information regarding the delivery methods of cannabis formulas and the most appropriate applications according to the condition and situation

-        Continually further our knowledge about new elements, characteristics, and behaviours of plant-based medicines.

-        Identify changes, patterns and other defined elements in order to assist in our work with any unique client phenomena.

The data collected for this purpose does not include any identifying information. In this case, only the following are recorded:

-        Client diagnosis / symptoms

-        Relevant demographic and psychographic information ie: Age, Location, Birth Sex

-        Recommended protocol, any changes to this protocol and why

-        Results (initial, short-term and long-term outcome)

Should you wish to opt out of this program, you have the right to request that your data is not disclosed in our findings, as per the statement of disclosure below:

-        Upon written request you have the right to receive a list of items this office disclosed about your healthcare information.

-        You have the right to request that this office place additional restrictions on the disclosure of your protected health information.

-        You have the right to request that we amend your protected health information.

Our Office intends to protect your personal data. We have implemented appropriate physical, administrative and technical safeguards to help us protect your personal data from unauthorised access, use and disclosure. For example, we encrypt certain personal data such as payment information when we transmit such information over the Internet. We also require that our subsidiary Certified Practitioners and/or associated Clinics. and service providers protect such information from unauthorized access, use and disclosure.
